Notice of Alleged Infringement (“Notice”)
If more than one copyrighted work is covered by this notice, please specify which copyrighted work you believe has been violated. Alternatively, you may offer a representative list of the copyrighted works you believe have been violated. Provide information reasonably sufficient to enable us to locate the material, including at a minimum, if applicable, the URL of the link displayed on the Site(s) where such material may be found, along with identification of the material that you claim is infringing (or to be the subject of infringing activity) and that is to be removed or to which access is to be disabled.
Please provide your phone number, email address, and mailing address. Include the following two sentences in the Notice’s body:
“I hereby state that I have a good faith belief that the disputed use of the copyrighted material is not authorised by the copyright owner, its agent, or the law (e.g., as a fair use),” the statement reads.
“I hereby state that the information in this Notice is accurate and, under penalty of perjury, that I am the owner, or authorised to act on behalf of the owner, of the copyright or of the exclusive right under the copyright that is allegedly infringed.”
Include your electronic or written signature as well as your full legal name.
